Navigating the journey to parenthood can often feel like deciphering a complex map, especially when treatment options like Intrauterine Insemination (IUI) come into play. While IUI is a relatively straightforward procedure, the medications used to stimulate ovulation beforehand are incredibly nuanced, each with its own unique characteristics and best-fit scenarios. It works by tricking your brain into thinking there isn't enough oestrogen, prompting the pituitary gland to release more Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH). These hormones then stimulate the ovaries to produce follicles, hopefully leading to ovulation.

Clomiphene is often my first choice for patients who are anovulatory or oligo-ovulatory, particularly those with PCOS, provided their fallopian tubes are open and their partner's semen analysis is within acceptable parameters. It's generally well-tolerated, cost-effective, and easy to administer at home.

  • Typical dosage: 50mg to 150mg daily for 5 days.
  • Potential side effects: Hot flashes, mood swings, headaches, visual disturbances (rare), thinning of the uterine lining (a key consideration).
  • Success rates: Varies greatly depending on underlying cause, but a significant proportion of women will ovulate. Pregnancy rates per cycle for IUI with Clomiphene are often in the range of 8-15%.
  • Cost: Generally very affordable, often under INR 500 per cycle for the medication itself, making it a popular choice in India.

Letrozole: The Modern Contender

Letrozole, marketed under names like Femara, is another oral medication that has rapidly gained popularity over the last decade, particularly for women with PCOS. Unlike clomiphene, letrozole is an aromatase inhibitor. It temporarily blocks the production of oestrogen, which again signals the brain to release more FSH and LH, leading to follicular growth. The key difference is how it works: it clears out of the system quickly, and generally doesn't have the same negative impact on the uterine lining or cervical mucus that clomiphene sometimes does.

I often prefer letrozole for my PCOS patients because it tends to result in a more predictable ovulatory response and a lower risk of multiple pregnancies compared to clomiphene. Its short half-life means it doesn't linger in the body, which is reassuring for many couples.

  • Typical dosage: 2.5mg to 7.5mg daily for 5 days.
  • Potential side effects: Hot flashes, fatigue, dizziness, mild headaches. Generally fewer and milder side effects than clomiphene.
  • Success rates: Comparable to or sometimes better than clomiphene, especially in PCOS patients, with IUI pregnancy rates per cycle often in the 10-20% range.
  • Cost: Slightly more expensive than clomiphene, typically ranging from INR 800 to INR 2000 per cycle for the medication.

Gonadotropins: The Potent Boost

When oral medications like clomiphene or letrozole aren't effective in inducing ovulation, or when a more robust follicular response is desired (for instance, in some cases of unexplained infertility or mild male factor infertility), we turn to injectable gonadotropins. These medications are synthetic versions of the natural hormones FSH (Follicle-Stimulating Hormone) and sometimes LH (Luteinizing Hormone), which directly stimulate the ovaries to produce multiple follicles.

Gonadotropins are much more potent than oral medications and require very close monitoring, including frequent ultrasound scans and blood tests, to prevent ovarian hyperstimulation syndrome (OHSS) and high-order multiple pregnancies. They are administered as subcutaneous injections, which many patients learn to do themselves at home after initial training. Regardless of the medication chosen, meticulous monitoring is paramount. This typically involves a series of transvaginal ultrasound scans to track follicular growth and uterine lining thickness, along with blood tests to measure hormone levels (oestradiol, LH). This allows us to time your IUI precisely, typically 24-36 hours after a 'trigger shot' (an HCG injection) that mimics the natural LH surge to induce final egg maturation and ovulation.

In India, as per the ART Act 2021, all fertility clinics must adhere to strict guidelines regarding patient care and safety.
